BACKGROUND: Fat embolism syndrome is a life challenge syndrome. Early diagnosing and treatment can significantly improve the patient’s prognosis likelihood of success. This occurs mainly after long bones fractures or orthopedic surgery up to 95% diagnosed cases, but in unusual situation be faced as well. These rare situations include diabetes mellitus, video-assisted thoracoscopies, fatty liver, fat injection plastic cosmetic procedures. The this increased if multiplex occur same patient simultaneously. usually manifested with respiratory changes (hypoxemia ARDS), neurological focal symptoms (confusion, headache, aphasia, hemiplegia), skin abnormalities (petechias, rush conjunctiva oral mucosa). clinical scenario begins typically 24−72 h injury, mainly, are first, followed by finally petechias most significant sign. CASE REPORT: In case, we report case unexpected soft-tissue minimal trauma. first that according literature, aim reporting emphasize happen perhaps every trauma even minor absence bone fractures. CONCLUSION: We strongly suggest should make physicians taking consideration fracture missed, early adequately treating patient, optimizing his chances survive.
